Saw Palmetto Side Effects: Stomach, Bleeding and PSA Notes
Saw palmetto side effects are usually mild and infrequent. NCCIH lists digestive upset, dizziness and headache in studies lasting up to three years. It does not appear to change PSA readings. The same page names no specific drug interactions, which is not proof there are none, so ask your doctor or pharmacist if you take medicine.

What are the common saw palmetto side effects?
The common ones are mild digestive upset, dizziness and headache. NCCIH calls the herb generally well tolerated in studies lasting up to 3 years (NCCIH, Saw Palmetto).
In a one-year trial of 225 men, side effects were similar on saw palmetto and on placebo (Bent et al., NEJM 2006). The Cochrane review reached the same view: adverse events were probably close to placebo (Cochrane review, Franco 2023).
That is reassuring, but it covers the trial products. Protoflow holds 84.5 mg of saw palmetto fruit among 14 actives, and no trial has tested the full capsule. Read the wider list on our Protoflow side effects page.
| Topic | What the source says |
|---|---|
| Stomach upset | Mild, infrequent |
| Dizziness, headache | Mild, infrequent |
| PSA readings | Does not appear to change them |
| Named drug interactions | None listed |
| Pregnancy, breastfeeding | May be unsafe |
Does saw palmetto cause bleeding or interact with blood thinners?
The sources we hold do not link saw palmetto to bleeding, and NCCIH lists no specific drug interactions. That is an absence of data, not a guarantee.
Supplements can still interact with medicines or raise bleeding risk, and you should tell every provider what you take, including before surgery (NCCIH, Using Dietary Supplements Wisely). Protoflow also holds ginkgo and vitamin E, which have their own bleeding notes. You can read about them in our guide to supplements and prostate medicines.
Ask your doctor or pharmacist before use if you take warfarin, apixaban or clopidogrel. We do not know your medicines, so we cannot call anything safe with them.
- Bring the bottle or the Supplement Facts panel to the visit.
- Report unusual bruising or nosebleeds that will not stop.
- Tell the surgeon about every supplement before an operation.
Does saw palmetto change a PSA test?
It does not appear to. NCCIH says saw palmetto does not seem to affect PSA readings, and the 2006 trial saw no PSA difference from placebo (NCCIH, Saw Palmetto) (Bent et al., NEJM 2006).
That matters because PSA is used to look for prostate problems. A steady PSA does not mean the herb helped, only that it did not mask the number. Tell your doctor you take it anyway.
Keep taking prescribed medicine. Do not swap a prescription for a herb, since the large trials found no benefit for urinary symptoms (Cochrane review, Franco 2023).

See packsWhen should you stop saw palmetto and call a doctor?
Stop and call if you have a rash, swelling, trouble breathing, or upset that does not pass. Get care right away for the red flags below.
Unable to pee, blood in the urine, fever with painful peeing, or severe lower belly pain need care right away.
A weak stream or waking at night to pee also needs a doctor's check, because the cause matters: an enlarged prostate, infection, diabetes or prostate cancer. A herb should never delay that visit.

FAQ
Saw palmetto safety questions
01Can saw palmetto cause headaches or dizziness?
Yes, both are listed by NCCIH as mild and infrequent effects in studies lasting up to three years. If either keeps returning, stop the product and tell your doctor or pharmacist what you take.
02Is saw palmetto safe during pregnancy?
NCCIH says it may be unsafe in pregnancy or breastfeeding. Protoflow is labeled for adults and aimed at men, so a pregnant or nursing woman should not use it without her doctor's advice.
03Do I need to stop saw palmetto before surgery?
Tell your surgeon about it well ahead of time. Supplements can raise bleeding risk or affect anesthesia, and NCCIH advises telling every provider. The surgeon, not this site, decides when to stop.
