Prostatitis vs BPH: Symptoms, Causes and Care
Prostatitis vs BPH is a question of cause. Prostatitis is swelling or infection of the prostate and often brings pain, while BPH is a noncancerous growth of the gland that mostly slows the stream. Both can cause frequent or weak peeing, so a doctor needs to tell them apart.

What is the difference between prostatitis and BPH?
Prostatitis is inflammation of the prostate, sometimes from infection. BPH is a noncancerous growth of the gland that squeezes the urethra.
NIDDK lists four types of prostatitis: chronic pelvic pain syndrome, acute bacterial, chronic bacterial and a type with no symptoms (NIDDK, Prostatitis). BPH affects about 5% to 6% of men aged 40 to 64 (NIDDK, Prostate Enlargement (BPH)).
| Point | Prostatitis | BPH |
|---|---|---|
| What it is | Inflammation or infection | Noncancerous growth |
| Pain | Common: pelvic, groin or lower back | Uncommon |
| Onset | Sudden or long-lasting | Gradual |
| Fever | Possible if bacterial | Not typical |
| Usual care | Antibiotics if bacterial; other options | Watchful waiting, medicines, procedures |
Which symptoms point to prostatitis?
Pain is the clue most linked to prostatitis. Chronic pelvic pain syndrome means pain or discomfort for three months or more between the scrotum and anus, in the lower belly, the penis or the lower back (NIDDK, Prostatitis).
Painful or frequent urination and a weak stream can appear too, which is why it gets mixed up with BPH. Acute bacterial prostatitis is sudden and severe, with fever, chills, painful urination and sometimes blockage, and it needs antibiotics (NIDDK, Prostatitis).
Pelvic or groin pain points toward prostatitis; a slow, weak stream alone points more toward BPH.
Weak stream, trouble starting and night trips are the usual BPH picture (NIDDK, Prostate Enlargement (BPH)). Our prostate basics guide explains how the gland sits around the urethra.
When do prostate symptoms need care right away?
Seek care right away if you cannot pee, see blood in your urine, have fever and chills with painful peeing, or have severe lower belly pain (NIDDK, Prostatitis). These apply to both conditions.
For milder, long-lasting symptoms, book a visit. A weak stream, straining or waking at night to pee can come from BPH, infection, diabetes or prostate cancer, so the cause matters.
- Sudden fever with painful peeing: urgent
- Cannot pee: urgent
- Pelvic pain lasting weeks: book a visit
- Slow stream for several days: book a visit

See packsDoes a prostate supplement help either condition?
No supplement has been shown to treat prostatitis or BPH. NCCIH finds saw palmetto probably not helpful for BPH (NCCIH, Saw Palmetto), and a Cochrane review found little or no difference in symptoms (Cochrane review, Franco 2023).
Protoflow is a dietary supplement with 14 labeled actives. It has not been tested in a trial, and it does not treat either condition. It is not a substitute for antibiotics or a checkup. Keep taking prescribed medicine, and ask your doctor or pharmacist about tamsulosin, finasteride or any other drug before adding capsules.
Our prostate health guides cover each topic in more depth.

FAQ
Prostatitis and BPH questions
01Can prostatitis turn into BPH?
No. They are separate conditions with different causes. A man can have both at the same time, though, which can make symptoms harder to sort out. A doctor can test for infection and check the size of the gland.
02Is prostatitis contagious?
Prostatitis is not contagious in the usual sense. Bacterial types come from germs reaching the prostate, often from the urinary tract, and many cases have no clear infection. Ask your doctor about testing if you also have other symptoms.
03How long does chronic prostatitis last?
Chronic pelvic pain syndrome is defined as pain lasting three months or more, and it can come and go. Care is tailored to you, so see a doctor for a plan instead of waiting for it to pass.
