How prostate supplements work is mostly unproven. Makers propose that plant compounds change hormone activity, calm inflammation or relax muscle around the urethra. Human trials of the main herb, saw palmetto, found little or no benefit. So these are ideas, not shown effects. A doctor should find the cause of any urinary symptom.

How do prostate supplements work in theory?
In theory they act in a few ways. Each is a proposal from lab or animal work, not a result seen in men.
The first idea is hormonal: that a plant compound slows the change of testosterone into a stronger form that drives prostate growth. The second is that plant compounds calm inflammation. The third is that they relax smooth muscle near the bladder neck, so urine flows more easily.
These ideas explain why a herb is sold. They do not show that a capsule does it in a person.

Do the proposed mechanisms show up in trials?
Mostly no. Good ideas in a dish often fail in people.
Saw palmetto is the clearest case. A year-long trial found no difference from placebo (Bent et al., NEJM 2006), and a Cochrane review of 27 trials agreed (Cochrane review, Franco 2023). The NIH center says it does not appear to change PSA (NCCIH, Saw Palmetto).
Tribulus is another. Of 10 studies, 8 found no change in male hormones (Vilar Neto et al., Nutrients 2025). A mechanism that does not change the marker it should change is hard to defend.
A plant that acts in a test tube may do nothing at a capsule dose, and nothing in a person.
Why does dose change how a prostate supplement works?
Dose decides whether any effect is possible. A herb at a fraction of a studied amount cannot be expected to act like the studied amount.
Take saw palmetto. The big trials used 320 mg a day or more (Barry et al., JAMA 2011 (CAMUS)). A label with 84.5 mg per serving holds about a quarter of 320 mg. Tribulus studies for sexual function used 400 to 750 mg (Vilar Neto et al., Nutrients 2025), and a label with 120 mg is well below that.
