BPH Symptoms: Hesitancy, Weak Stream and Frequency
BPH symptoms are urinary changes caused by an enlarged prostate pressing on the urethra: trouble starting, a weak or stop-start stream, a feeling the bladder is not empty, and needing to go often or urgently, including at night. They can also come from other causes, so a doctor should check them.

What are the common BPH symptoms?
The common BPH symptoms are hesitancy, a weak stream, dribbling, a sense of incomplete emptying, frequency, urgency and waking at night to pee (NHS, Enlarged Prostate). Doctors group them as lower urinary tract symptoms, or LUTS.
BPH is a noncancerous growth that affects about 5% to 6% of men aged 40 to 64 and 29% to 33% of men 65 and older (NIDDK, Prostate Enlargement (BPH)). Our prostate basics guide explains the anatomy.
| Symptom | What it feels like |
|---|---|
| Hesitancy | You wait or strain before the flow starts |
| Weak stream | Slow flow, or it stops and starts |
| Incomplete emptying | The bladder still feels full |
| Frequency and urgency | Going often, or needing to go right now |
| Nocturia | Waking at night to pee |
Why does an enlarged prostate cause a weak stream?
The prostate wraps around the urethra, the tube that carries urine out. When it grows, it can squeeze that tube and make the bladder work harder.
Size does not always match how bad the symptoms are (NIDDK, Prostate Enlargement (BPH)).
- 1. Bladder
Stores urine. When it squeezes, urine flows down the urethra.
- 2. Prostate
A walnut-sized gland wrapped around the urethra just below the bladder.
- 3. Urethra
The channel urine flows through. The prostate surrounds its first part.
- 4. Enlarged prostate (BPH)
As the gland grows, it can squeeze the channel: a weak stream, straining, getting up at night.

Which BPH symptoms need a doctor right away?
Unable to pee, blood in the urine, fever and chills with painful peeing, and severe lower belly pain need care right away (MedlinePlus, Enlarged Prostate).
Do not wait for an appointment if any of these four signs appear.
Milder symptoms still need a check. A weak stream or waking at night to pee can be BPH, infection, diabetes or prostate cancer, and treatment differs. BPH does not raise prostate cancer risk, but symptoms alone cannot rule cancer out (NHS, Enlarged Prostate).

FAQ
BPH symptom questions
01What does LUTS mean?
LUTS stands for lower urinary tract symptoms: problems with storing or passing urine, such as a weak stream, urgency or frequency. The term describes symptoms, not a cause, so BPH is only one possible reason for them.
02Can other medicines make urinary symptoms worse?
Yes. The NHS advises checking whether any medicine you take is worsening symptoms. Do not stop a prescribed medicine yourself. Ask your doctor or pharmacist to review the list, including supplements.
03Is a feeling of incomplete emptying always BPH?
No. It can come from BPH, but also from infection, bladder problems or other conditions. Because the causes differ in seriousness, a doctor should check it instead of leaving you to guess.
